The National Registry Emergency Medical Technician (NREMT) cognitive exam is a computer adaptive test (CAT) that assesses the competency of candidates seeking certification as an EMT-B. The exam consists of a variable number of items, ranging from 70 to 120. Among these items, 60 to 110 are 'live' and contribute to the final score, while the remaining 10 serve as pilot questions and do not affect the candidate's outcome. The exam must be completed within a maximum time limit of 2 hours.

The exam comprehensively covers various aspects of EMS care, including Airway, Respiration & Ventilation; Cardiology & Resuscitation; Trauma; Medical; Obstetrics/Gynecology; and EMS Operations. The majority of items focus on adult and geriatric patients, accounting for 85% of the content, while the remaining 15% addresses pediatric patients. To pass the exam, candidates must meet the prescribed standard level of competency, which involves demonstrating the ability to deliver safe and effective emergency medical care at an entry-level capacity.
